WHO SUBMITTED APPLICATION FORMS?

Both members (student and administrative representative) of the nominated LSAMP Center team had to submit an RTP Application as part of the application documents to be considered for participation in the LSAMP at the Smithsonian '01 Winter Workshop.



WHY WAS THE APPLICATION FORM REQUIRED?

Information was needed from applicants to expedite the preparations for hosting participants in the January workshop.  This information was used as part of the evaluation process to select the 20 teams and to complete the necessary forms and finalize agenda topics.




TECHNICAL NOTES ABOUT ADDING TEXT TO THE ON-LINE FORM

For on-line application categories requiring text entries, the text could be entered directly into the space provided or composed separately in a word processing system and "copied" and "pasted" directly into the appropriate section of the form.

Be aware that not all web browsers offer the word wrap option compatible with the RTP site programming, so text entered directly may have appeared as one continuous sentence. However, all text entered will print and be viewed electronically by selection panels as complete, formatted documents, regardless of how it may appear on-line to the applicant.

Due to special indexing, some fields only accepted entries from the suggested list. If the most appropriate answer was not included in the listing of options, applicants selected the next best choice from the list but offered better answers as part of Optional question #9 at the end of the application form.




SELECT PROGRAM

The on-line application for the LSAMP at the Smithsonian '01 Winter Workshop utilized the same application forms as the Research Training Program summer session.  To direct materials to the appropriate destination, applicants had to make sure to click the "other" option for item #1 and indicate "LSAMP Winter Workshop" in the "please specify" section. 

Identifying the appropriate program was critical to directing materials to the electronic correct file.




EDITING OR MODIFYING APPLICATION INFORMATION

All application information for the LSAMP at the Smithsonian '01 Winter Workshop entered on-line at the RTP web site could be modified, or completely replaced, until 1 December 2000. After the application deadline, modify functions automatically turned off and new entries or modifications were not added to the current year files.

NOTE: It was not possible to retrieve previous text entries once new text was accepted by the system!




PERSONAL IDENTIFICATION NUMBER

To route application materials from the on-line electronic submission option to each nominee's personal application file, all applicants had to select and enter a unique 9-digit number (e.g. social security number). This number serves as an Application Identification Number electronically linking all materials submitted and allowing access to previously entered data. This same EXACT number had to be used by LSAMP Center Directors to submit the letter of recommendation and by applicants to review or update application documents.

The electronic system records the number exactly as entered the first time an applicant registers their file including the use of dashes, dots, spaces, etc.  Because this can become unnecessarily problematic, applicants were encouraged to use only numeric characters and avoid other elements in the Applicant Identification Number.

Applicants who did not submit materials electronically, and did not identify an Applicant Identification Number, were randomly assigned a number.




LSAMP at the SMITHSONIAN '01 WINTER WORKSHOP APPLICATION FORM CATEGORY INSTRUCTIONS:

To ensure successful review, all nominees for the LSAMP at the Smithsonian '01 Winter Workshop had to provide answers for information requested on the RTP Application form. Most RTP Application Form categories are self-explanatory. Further explanation is provided here for reference. 

       1.  Select Program: All applicants to the '01 Winter Workshop must click the "other" option and record "LSAMP Winter Workshop" in the "please specify" section. This information determines the review set for processing application materials. It is critical to enter this to ensure proper processing of your application documents. If you have questions, contact the system administrator. 

       2.  Name: Identifies the applicant's full, correct name. To assure accurate grouping of items, applicants must use a consistent recording of their name for all application materials. Please insure that references also use the applicant's name as recorded heselection process to contact the applicant. 

       3.  Current Address: Identifies the applicant's current mailing address. 

       4.  Current Contact Information: Identifies the applicant's current telephone and e-mail information. This information will be used throughout the

       5.  Social Security Number (SSN): Used to identify the applicant, especially when applicants share the same name or to match application materials for applicants using different names or name combinations. Applicants not holding a SSN, or not wishing to provide a SSN, may simply record n/a. 

       6.  Date of Birth: Used to further identify the applicant, especially when applicants share the same name or to match application materials for applicants using different names or name combinations.

       7.  Permanent Address: Identifies the applicant's permanent mailing address and contact information. This information is particularly important to locate and notify applicants of immediate changes during "winter semester break" and of new developments after the standard dates. 

       8.  Permanent Contact Information: Identifies the applicant's permanent contact information. This information is particularly important to locate and notify applicants of immediate changes during "winter semester break" and of new developments after the standard dates. 

       9.  Citizenship: Identifies the applicant's citizenship. US citizens are asked to identify their state of residence. Non-US citizens are asked to identify their country of citizenship, immigration status and/or visa information.  Non-US citizens must also identify their place of birth. 

      10.  University: Identifies the name of the academic institution the applicant is currently attending or affiliated with. To further identify the institution, location (city, state, and country) is requested. 

      11.  Academic Advisor: Only for students.  Identifies the applicant's academic advisor and contact information where the person can be contacted for consultation.  Applicants who do not have an academic advisor, or applicants not wishing to disclose the name of their advisor, may simply record n/a. 

      12.  Academic Level: Only for students.  This references the US standard academic classification system of a four-year undergraduate career. Applicants enrolled with a university that follows a different academic classification are asked to identify the year equivalent or explain their classification compared to the US standard. 

      13.  Academic Major(s)/Minor(s): Only for students.  Identifies the undergraduate degree(s) anticipated by the applicant. 

      14.  Grade Point Average: Only for students. Identifies the evaluation scheme of the applicant's academic institution. For applicants enrolled at institutions employing non-traditional evaluation schemes, an explanation may be provided as supplementary information. Generally, with few university courses from which to compute a GPA, students in their first or second undergraduate year are also asked to identify their high school evaluation level. More senior applicants, who have taken advanced courses related to their major area of study are asked to also calculate and enter their current GPA for courses specifically related to their declared major. 

      15.  Career Goals:

- - For students, provide a short description of your career goals including education, particular disciplines and techniques of interest.

- - For LSAMP Center representatives, provide a short description of your official duties within the LSAMP network. 

      16.  Natural History Discipline: Identifies the particular discipline within the NMNH system that you are most interested in being assigned to during the shadowing experience.  For sorting purposes, only one discipline may be selected. 

      17.  Advisor/Mentor: Identifies the scientist from within the NMNH community that the applicant is most interested in shadowing. Additional advisor/mentor listings provide insight into the applicant's preferences for other options if the first choice is not available. 

      18.  Skills: Only for students.  Identifies the skills, methods, and techniques accomplished (proficient/mastered) by the applicant relevant to natural history disciplines. 

      19.  Academic Achievements, Honors, and Awards: Identifies achievements, honors and awards earned by the applicant. 

      20.  Courses and Grades. Only for students.  Identifies the applicant's academic background (courses) and evaluation (grades). An official or unofficial transcript may be substituted. 

      21.  Professional Experience: Identifies the professional experience and background of the applicant including membership in professional societies, presentations, publications, and research experience. A further explanation may be included as part of the cover letter. 

      22.  References: Record the LSAMP Center Director as one option and, if applying from a partner organization, record the chief LSAMP liaison of the partner institution

      23.  Additional Survey Information: All information is OPTIONAL except item #8.  All applicants must identify their LSAMP Center affiliation.  To review the recognized listing, click here.  Note:  all applicants selected for participation in the '01 Winter Workshop will receive the same award package including provided housing, transportation to/from Washington D.C. plus a per diem of $60 per day for the 6 days of the workshop.
